New Patients
Healthmark Foot and Ankle Associates is a private medical/surgical practice limited to the care of diseases and injury of the foot and ankle. We provide complete medical, orthopedic and surgical services including prevention, diagnosis, treatment and management of disorders of the nails, skin, foot, ankle and related structures of the leg.
We cater to all segments of our community including adults, children, senior citizens, those suffering from birth deformities, diabetes, and benign or malignant tumors. Our doctors have extensive experience with sports related injuries and impairments resulting from industrial accidents for which workers' compensation is provided.
For problems outside the scope of podiatry, Healthmark Foot and Ankle Associates will gladly help you locate an appropriate physician. If you already have a family physician, we will forward all the notes and lab results at your request to keep your doctor informed of your progress
Appointments
Appointments may be arranged Monday through Friday. Early morning and evening appointments are available. Occasionally we must see an emergency patient, which may delay your appointment. Whenever possible we will notify you of any delay. We arrange our schedule around pre-arranged appointments and request that you inform us if you foresee any changes or delays to your visit. A missed appointment fee is charged to you if you fail to inform us of a cancellation 24 hours prior to your scheduled 
appointment.
In the event of an emergency, please go to your nearest hospital emergency room. The attending physician may contact us as necessary.
Feel free to call if you have a medical question or problem. If the doctor is not available, our staff will relay the information. Your call will be returned at his earliest opportunity. After normal hours a 24 hour answering service will forward your message. Please state your question or problem clearly and the doctor will get back to you as soon as possible.
If you are dissatisfied with your care or have a question regarding your fee or service, feel free to talk with us about it. We will make every effort to answer questions frankly, rectify any injustices or address any misunderstandings.
Managed care patients must present a referral form whenever they see a specialist. Please contact your primary physician in advance to obtain the necessary referral for your visit. Referrals cannot be backdated.
Insurance
Our fees are based on usual and customary charges and are locally competitive. Fees for an initial visit or a visit for a new problem may be higher than routine care visits. If you have any questions regarding our fees, please feel free to discuss them with us.
Office visits may or may not be covered by insurance. Fees for non-surgical care, surgery and covered x-ray services are billed for you as a courtesy, and include usual follow up care but do not include non-covered x-rays, casts, splints or orthotics. Payment in full for surgical supplies, equipment and orthotics is due at time of dispensing. If payments due will be delayed, please contact us so that we can try to arrange a mutually acceptable payment plan.
Co-payment due is always required when service is rendered. Please bring your insurance card to each visit and pay your copayment at that time. If you fail to pay your co-pay at the time of your visit a surcharge will be applied to your account.
You are responsible for payment of all bills regardless of contractual arrangements with your insurance plan or attorneys in pending litigation. Patients with Medicare coverage or medical plans offering complete coverage may not be required to pay additional out of pocket expenses. We will do everything reasonably possible to assure that you receive the full benefits provided under your plan.
